HOW TO COMPLETE THIS ENROLLMENT FORM
Thank you for your interest in the Rock School of Ministry! It is our intention that the enrollment process serves as a valuable tool in helping you prayerfully evaluate whether this is the right program and time for you.
Please answer all questions. Husbands and wives enrolling as students must complete separate applications.

Please answer the questions in a Word.doc or Word.docx file and attach it at the end of this section.
A non-refundable fee of $50 is required upon submission of your enrollment form. This fee covers the cost of processing your form.
Tuition-$2450 includes retreats, materials and coaching sessions.
Due to some of the physical aspects of the Rock School of Ministry, a Medical Report Form is included for your completion. This information will be used to ensure your safety and is completely confidential.
After submitting your Enrollment Form, please ensure you have reference forms completed by your Church leader, employer, and Christian friend. Please go to rockschoolofministry.com/enrollment and click on step 2 "Connect With Your References" to find information to send to your references.
